Robotically assisted radical laparoscopic prostatectomy in a patient with ectopic ureteral insertion into the prostate

چکیده
Robotically assisted laparoscopic radical prostatectomy (RaLRP) has become an alternative surgical treatment option for patients with localized prostate cancer. Rare anatomic variations result in modification of established techniques. We describe the first successful completion of RaLRP for localized prostate cancer in a patient with coexisting complete left ureteral duplication and ectopic insertion of the upper pole moiety ureter into the prostatic urethra. After standard radical prostate excision and vesicourethral anastomosis, the unique anatomy in this case required additional reconstructive maneuvers including construction of a Y-type conjoined ureteral anastomosis and ureteroneocystotomy. The procedure was safely completed without open conversion. Operative time was 420 min and EBL was 200 mL. Ureteral stents were removed in six weeks and IVP showed no evidence of obstruction. When faced with a ureteral anomaly, RaLRP can be safely completed. Robotic technology is helpful in performing additional delicate reconstructive maneuvers.

Re: Comparison of radical prostatectomy techniques: open, laparoscopic and robotic assisted.
This timely paper reviews the current status and respective roles of laparoscopic, robotically-assisted and open radical prostatectomy in the management of localized prostate cancer. While open radical prostatectomy remains the gold standard of treatment, a minimally-invasive approach has been available since 1997 in the form of laparoscopic radical prostatectomy. Minimally-invasive approaches ...

O ver 75,000 radical prostatectomies were performed in the USA last year for the treatment of prostate cancer. Most of these were performed by radical retropubic prostatectomy (RRP), the gold standard for treatment of this disease. However, the quest for increased efficacy and decreased morbidity is having as profound an impact on the treatment of prostate cancer as in any other area of medicin...

We evaluate the use of robotically manipulated transrectal ultrasound (TRUS) for real-time monitoring of prostate and periprostatic anatomy during robot-assisted prostatectomy (RAP). Ten patients with clinically organ-confined prostate cancer undergoing RAP underwent preoperative and real-time intraoperative biplanar TRUS evaluation using a robotically manipulated TRUS device (ViKY System; Endo...
متن کاملA Case of Robot-Assisted Laparoscopic Radical Prostatectomy in Primary Small Cell Prostate Cancer
Primary small cell carcinoma of the prostate is a rare and very aggressive disease with a poor prognosis, even in its localized form. We managed a case of primary small cell carcinoma of the prostate. The patient was treated with robot-assisted laparoscopic radical prostatectomy and adjuvant chemotherapy. Herein we report this first case of robot-assisted laparoscopic radical prostatectomy perf...
متن کاملRobotic “Side Hole” Technique Placement of Ureteral Stent for Ureteral Lesion
A 67-year-old patient underwent robotic-assisted laparoscopic radical prostatectomy and experienced right ureteral lesion. The laceration was recognized intraoperatively and immediately repaired over the ureteral double J stent. The wire and the stent were first advanced distally to the bladder. Then the wire was pulled with its soft end, cranially through one of the side holes of the proximal ...
